By providing my signature below, I hereby authorize Third Oak Productions LLC, to charge the credit card that I have provided and associated with my account, for monthly recurring fees for website maintenance services.
I understand I receive only one (1) hour of website updates and WordPress website maintenance per month. If I need more hours per month, additional hours can be purchased at a reduced rate.
I understand and agree that this authorization will remain in effect until I cancel it in writing, and I agree to notify Third Oak Productions LLC in writing (via email) of any changes in my account information or termination of this authorization at least 30 days prior to the next billing date.
If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day.
I certify that I am an authorized user of this credit card and will not dispute these scheduled transactions with my credit card company, so long as the transactions correspond to the terms indicated in this authorization form.
I understand that to discontinue these charges, I must submit a formal cancellation request to Third Oak Productions LLC via email or through this contact form here, and receive a confirmation of cancellation from Third Oak Productions LLC.
I understand and accept that no refunds will be given for any charges made prior to the date of cancellation. It is my responsibility to ensure the cancellation request has been received and acknowledged by, Third Oak Productions LLC.
